Great work!  I'll post suggestions in this community.  We'll use it as the default taxonomy in our OpenHMIS http://openhmis.org , and after some actual use, we'll have some more useful feedback.  Thanks for all your hard work!

I was thinking a standards development organization would integrate them through a process. Something hosted by OASIS, or NIEM, or ...?  That way, if any stakeholder/actor needs a new term, it could be added through a set of predefined and impartial steps, based solely on the merits of the changes requested.
