#Gerontological #nursing is the specialty of nursing pertaining to older adults. Gerontological #nurses work in collaboration with older adults, their families, and communities to support healthy aging, maximum functioning, and quality of life. The term gerontological nursing, which replaced the term #geriatric nursing in the 1970s, is seen as being more consistent with the specialty's broader focus on health and wellness, in addition to illness.

Gerontological nursing is important to meet the health needs of an aging population. Due to longer life expectancy and declining fertility rates, the proportion of the population that is considered old is increasing. Between 2000 and 2050, the number of people in the world who are over age 60 is predicted increase from 605 million to 2 billion. The proportion of older adults is already high and continuing to increase in more developed countries. In 2010, seniors (aged 65 and older) made up 13% and 23% of the populations of the #US and #Japan, respectively. By 2050, these proportions will increase to 21% and 36%.

Gerontological nursing draws on knowledge about complex factors that affect the health of older adults. Older adults are more likely than younger adults to have one or more chronic health conditions, such as #diabetes, #cardiovascular disease, #cancer, #arthritis, hearing impairment, or a form of #dementia such as #Alzheimer's disease. As well, drug metabolism changes with aging, adding to the complexity of health needs.

Gerontological nurses work in a variety of settings, including acute care hospitals, rehabilitation, #nursinghomes (also known as long term care homes and skilled nursing facilities), assisted living facilities, retirement homes, community health agencies, and the patient's home. Depending on the conditions of the geriatric's health determines what type of facility one should reside in. Assisted living facilities are also known as a senior retirement home that provides care services depending on health conditions. Skilled nursing otherwise known as a nursing home is a place where they can reside and get provided with 24/7 cares.

Older adults have been referred to as "the core business of healthcare" by gerontological nursing experts. Population aging and the complexity of health care needs of some older adults means that older adults are more likely than younger people to use health care services. In many settings, the majority of patients are older adults. Thus, experts recommend that all nurses, not only those identified as gerontological nurses, need specialized knowledge about older adults. This position was endorsed by 55 US nursing specialty organizations.

#Behavioral #medicine is an interdisciplinary field combining both medicine and psychology and is concerned with the integration of knowledge in the biological, behavioral, psychological, and social sciences relevant to health and illness. These sciences include #epidemiology, #anthropology, #sociology, #psychology, #physiology, #pharmacology, #nutrition, neuroanatomy, #endocrinology, and #immunology. The term is often used interchangeably, but incorrectly, with health psychology. The practice of behavioral medicine encompasses health psychology, but also includes applied psychophysiological #therapies such as biofeedback, hypnosis, and bio-behavioral therapy of physical disorders, aspects of occupational therapy, rehabilitation medicine, and physiatry, as well as preventive medicine. In contrast, #health psychology represents a stronger emphasis specifically on psychology's role in both behavioral medicine and behavioral health.

Behavioral medicine is especially relevant in the #UnitedStates, where many of the health problems are primarily viewed as behavioral in nature, as opposed to medical. For example, smoking, leading a sedentary lifestyle, and alcohol abuse or other substance abuse are all factors in the leading causes of death in the United States. Practitioners of behavioral medicine include appropriately qualified #nurses, social workers, #psychologists, and #physicians (including medical students and residents), and these professionals often act as behavioral change agents, even in their medical roles.

A #biosimilar (also known as follow-on biologic or subsequent entry biologic) is a biologic #medical product which is almost an identical copy of an original product that is manufactured by a different company. Biosimilars are officially approved versions of original "innovator" products, and can be manufactured when the original product's patent expires.] Reference to the innovator product is an integral component of the approval.

Unlike with generic #drugs of the more common small-molecule type, biologics generally exhibit high molecular complexity, and may be quite sensitive to changes in manufacturing processes. Follow-on manufacturers do not have access to the originator's molecular clone and original cell bank, nor to the exact fermentation and purification process, nor to the active drug substance, although they do have access to the commercialized innovator product. Overall, it is harder to ascertain #fungibility between generics and innovators among biologics than it is among totally synthesized and semisynthesized drugs, which is why the name "biosimilar" was coined to differentiate these drugs from small-#molecule generics. A simple analogy is that it is harder to say that two wines are "sufficiently interchangeable", because of differences in yeast strain, weather, grape harvest, or terroir, than it is to say that two soda pops are "sufficiently interchangeable" because they contain the same flavoring powder and salts.

Drug related authorities such as #European #Medicines Agency (#EMA), Food and Drug Administration (#FDA), and #Health #Canada hold their own guidance on requirements for demonstration of the similar nature of two biological products in terms of safety and efficacy. According to them, analytical studies that demonstrate that the biological product is highly similar to the reference product notwithstanding minor differences in clinically inactive components, animal studies (including the assessment of #toxicity), and a clinical study or studies (including the assessment of #immunogenicity and #pharmacokinetics or #pharmacodynamics) are sufficient to demonstrate safety, purity, and potency in one or more appropriate conditions of use for which the reference product is licensed and intended to be used and for which licensure is sought for the biological product.

In case of a monoclonal antibody-containing medicinal product, such as #Remsima, extensive physicochemical and biological characterization for it and its reference product #Remicade was conducted in order to demonstrate their highly similar properties. Consequently, EMA has granted a marketing authorisation for only a few biosimilars since 2006 including a #monoclonalantibody which is recently approved. Meanwhile, on March 6, 2015, the FDA approved the United States's first biosimilar product, the biosimilar of filgrastim called filgrastim-sndz (trade name Zarxio) by #Sandoz.
