Dear CSSA at University of Texas Medical Branch at Galveston,

I am seeking your help with my postdoctoral research entitled: "Chinese Housing in North America: An Investigation of the Architectural Culture."

The study explores the interrelations of health, happiness, and housing of ethnic Chinese (including both renters and homeowners) who are living, studying, or working in North America, and the method is using an online survey. The link is: https://www.surveymonkey.com/s/healthyhappy

The results of the research will be published in a book. I aim to collect 1000 responses.

United Chinese Plan-Student Injury and Sickness Insurance Plan for ACSA - New York Service Center for Chinese Study Fellows 
2012-2013

New York Service Center for Chinese Study Fellows is pleased to offer an Injury and Sickness Insurance Plan underwritten by UnitedHealthcare Insurance Company to its members. International students, scholars, exchange program participants, participating in Optional Practical Training, internships, research and teaching, with a valid passport and all types of
visas that allow for study who have not applied for permanent residency in the U.S. are eligible to enroll in the plan on a voluntary basis. Eligible Dependents, as defined in the policy, who accompany the student and have a similar visa or passport, are eligible to enroll in the plan on a voluntary basis.

Highlights of the Coverage and Services offered by UnitedHealthcare StudentResources are:
● Up to $1,250,000 Per Insured Person Per Policy Year Maximum Benefit for Covered Medical Expenses.
● $500 Deductible per Insured Person per Policy Year for Preferred Providers, $1,250 Deductible per Insured Person per Policy Year for Out of Network Providers.
● Covered Medical Expenses for Preferred Providers are payable at 70% of Preferred Allowance and Out of Network benefits are payable at 50% of Usual and Customary charges (all benefits are subject to satisfaction of the Deductible, specific benefit limitations, maximums and copays as
described in the policy).
● Preferred Provider Out-of-Pocket Maximum of $7,500 Per Insured Person, Per Policy Year. After the
Preferred Provider Out-of-Pocket Maximum has been satisfied, covered medical expenses will be paid at 100% up to the policy Maximum Benefit subject to any applicable benefit maximums. Refer to the plan certificate for details about how the Out-of-Pocket Maximum applies.
● Out-OF-NETWORK SERVICES: After the $1,250 Deductible has been satisfied, Covered Medical Expenses will be paid at 50% of Usual & Customary Charges until the Insured Person has paid $12,000 in out of pocket expenses. Once the Insured Person has paid $12,000 in out of pocket expenses, the Company will pay 80% of Usual & Customary Charges for additional Covered Medical Expenses up to the $1,250,000 Maximum Benefit.
● Prescription Drug Benefits: $25 Copay for Tier 1 / $40 Copay for Tier 2 / $60 Copay for Tier 3 up to a 31-day supply per prescription filled at a UnitedHealthcare Network Pharmacy (UHPS). Prescriptions must be filled at a UHPS network pharmacy.
● Preventive Care Services which include, but are not limited to, annual physicals, GYN exams, routine screenings and immunizations are covered at 100% with no copay or deductible only when the services are received from a Preferred Provider. Please see www.healthcare.gov for complete
details of the services provided for specific age and risk groups.
● Coverage available for eligible Dependents.

● Scholastic Emergency Services – International Students are covered worldwide except in their home country. Services are provided for Medical Evacuation and Repatriation.

AS the Chinese New Year is coming closer, we, CSSA at UTMB, would like to extend our sincere new year wishes to you and your family.

We know it is always good to gather together with your friends, and family to celebrate this most important holiday for Chinese and many other Asian. This year, we, CSSA at UTMB, try our best to provide you this opportunity on the island. We will host the dinner at Healthy Chinese Buffet 6:30 PM Friday, Feb 8.

It is our first time that invite so many people in our community and at UTMB to a restaurant together. Due to limited sources of financial support, we only can support limited number of people. So please RSVP as soon as possible. First RSVP, first served! Please use the RSVP form below to RSVP. You will get confirmation by Feb 7 (Thurs).
