Combining IVC and LUS in mechanically ventilated patients

A growing body of research has established that using dynamic measures to determine fluid responsiveness is proberbly the optimal approach to guide fluid resuscitation. 

This firstly paper explore the physiologic rationale and current literature supporting the use of IVC diameter, distentability, collapsabilty as a marker of fluid responsiveness and lung ultrasound (LUS)  to identify the early pulmonary edema that develops when patients are overresuscitated.  

Then a decision tool that integrates IVC measurements and LUS to qualitatively guide fluid administration decisions is provided.

The authors convey years of anesthetic experience from R Adams Cowley Shock Trauma in Baltimore with practical tips on how provide anesthesia alongside massive transfusion and damage control surgery. The aim not only being optimized hemodynamics but also to improve micro circulatory flow and prevent endothelial damage, glycocalyx break down and fibrinolysis.
