What is Orthognathic Surgery?
Orthognathic or jaw surgery involves an operation to one or both of the jaws to alter their position and/or shape. It is usually performed to correct a major discrepancy of the size or position of the jaws in non-growing individuals.

Who performs the surgery?
Prior to the start of orthodontic treatment, or shortly after orthodontic appliances are placed, you should be seen by an oral and maxillofacial surgeon so that he or she can evaluate your condition and advise you as to the risks and benefits of orthognathic surgery. The surgery is performed by an Oral and Maxillofacial Surgeon. These specialist practitioners have training in dentistry, medicine, general surgery and head and neck surgery. In addition, a decision must be made regarding the status of your third molars (wisdom teeth). Typically, the wisdom teeth in the involved jaw(s) should be removed at least six months prior to surgery.

Who needs Orthognathic Surgery?
Orthognathic surgery is restricted to adults where skeletal growth has ceased. The surgery is undertaken when there has been a significant jaw developmental imbalance or asymmetry and orthodontic treatment alone cannot produce a satisfactory outcome. Braces will only move the teeth and have very little effect on jaw position or size. The most common situations requiring orthognathic surgery are:
 The lower jaw is too small and is set too far back. Surgery is usually performed to bring the lower jaw forward by increasing its length.
 The lower jaw is too big and is set too far forward. In these cases surgery is often required to reduce the prominence of the lower jaw and advance the position of the upper jaw.
 The lower jaw is diverging from the upper jaw resulting in an openbite or lack of vertical overlap of the front teeth. Surgery is usually performed on the upper jaw to allow the lower jaw to fully close against the upper.
 Facial asymmetry or deformities resulting from genetic, developmental and traumatic influences.
