The FDA released its review of Merck’s sleep aid – suvorexant. Data showed that certain doses of the drug led to increased daytime drowsiness and were associated with increased risk of suicidal thinking.

NIH guidelines specify that cognitive behavioral therapy should be the first line option for management of insomnia. New England Journal of Medicine "The Underside of the Silver Tsunami — Older Adults and Mental Health Care" Emphasizes the importance of broadly available, integrated behavioral care.

"Potential solutions might capitalize on older adults' increasing use of Internet-based and mobile health technologies, which offer opportunities to provide screening and treatment that might otherwise be inaccessible, or not sought, owing to lack of transportation, limited mobility, or stigma." We are delighted to say that we have had great success with this though our computerized cognitive behavioral therapy tools at Cobalt Therapeutics and have seen more interest in this from various Medicare organizations including primary care groups, PACE programs and Medicare Advantage programs.  It is incredibly rewarding to offer options that are making a difference.

An interesting article on present need and opportunity for behavioral health integration: "Several new initiatives, from patient-centered medical homes to accountable care organizations, create incentives for providers to ensure better outcomes for all of their patients. Given the statistics cited earlier, it is clear that providers must integrate mental health services if they are to be successful under these new models."

In this study, elderly nursing home residents taking non-benzodiazepine hypnotics such as zolpidem (Ambien), eszopiclone (Lunesta), and zaleplon (Sonata) to treat insomnia had a 70% increased risk of hip fractures.
