The Centers for Medicare and Medicaid Services reported that in 2009 US healthcare spending increased 5.6% to $2.47trillion and represented 17.3% of the US economy. The increase from 2008 to 2009 is the largest jump in healthcare spending since 1960, and it is remarkable that in a year in which the US economy contracted by 2.4% healthcare and the federal government were the only sectors to show an increase. This growth is also occurring from a high base, starting at a level 30% to 50% higher than most industrialized countries, and is unsustainable.
Part of the healthcare cost problem is the way that most healthcare services are provided. Hospital Care represents 31.1% of all healthcare costs and is notoriously expensive. The average cost for 1 day in the hospital in 2002 was $1,290, nearly double the average cost from 1990 and since 2002 hospital costs have increased by more than 50%. Professional Services, which in the Center for Disease Control (CDC) data includes Physician and clinical services, Dental services and other professional services represents another 31.3% of total healthcare costs and Nursing home care a further 5.9%. All of these services are provided onsite, in facilities and environments that are costly to maintain and where costs are increasing rapidly and represent more than 68% of the total healthcare expenditure in the US in 2007.

Bad Breath and Proton Pump Inhibitors

Many people are confused about the relationship between bad breath and proton pump inhibitors: do these drugs for gastroesophageal reflux disease (GERD) cause halitosis? The confusion arises from the fact that GERD itself is thought to be a cause of halitosis.
GERD is often cited as a cause for halitosis and proton pump inhibitors are a standard treatment for GERD-the idea that these drugs may cause the bad breath, or make it worse is a relatively new development. Some medical professionals, in fact, will assume halitosis in a patient with acid reflux is caused by the condition and use GERD drugs to treat it. We need more research to investigate proton pump inhibitors and bad breath to clarify these issues.
In 2006, Brazilian researchers reported on a study that suggested a link between bad breath and proton pump inhibitors. They reported that a large percentage of patients taking these drugs have halitosis. Furthermore, their research showed a relationship between halitosis and proton pump inhibitors regardless of whether the breath odor originated in the mouth or from a non-oral source. They suggested that the underlying cause was an overgrowth of bacteria, in either the mouth or the intestine, caused by the drug.
Interestingly, many experts feel that relatively few cases of halitosis are caused by acid reflux and odors traveling up the throat from the stomach. This lends more support to the suggestion that halitosis and proton pump inhibitors may be linked: it is possible some cases of bad breath associated with GERD in the past were actually caused by the drugs instead. In addition, it suggests that treating halitosis in GERD patients with these drugs may actually make the problem worse. The answers remain far from clear: more research on bad breath and proton pump inhibitors is required.
For the present, when patients suffer from bad breath and proton pump inhibitors are necessary for the treatment of GERD, the best recourse is still a good breath product that targets the bacteria that produce bad odors in the mouth. At least for those whose halitosis originates in the mouth, this approach should offer some relief.
Patients suffering from either halitosis or GERD should consult a qualified medical professional to determine the best course of treatment.

Budidaya Penangkaran Ayam Bekisar Sentolo
Ayam bekisar adalah hasil perkawinan antara ayam hutan hijau jantan (Gallus varius) dan ayam kampung/ayam buras betina (Gallus gallus domesticus).
Ada tiga tipe ayam bekisar, yaitu :
1. Gallus aenus yang berjengger bergerigi 8 kecil, pial berukuran sedang, warna bulu pada lapisan atas ungu dengan plisir kuning emas.
2. Gallus temminckii memiliki jengger bergerigi enam, pial berwarna jambu, bulu merah mengkilap dan berplisir merah kecoklatan.
3. Gallus violaceus dengan jengger bergerigi bagus, ukuran pial sedang, warna bulunya ungu dengan permukaan yang halus.
Ayam bekisar memiliki ukuran lebih kecil dibandingkan ukuran ayam kampung jantan, tetapi lebih besar daripada induk jantannya. Warna bulunya hitam kehijauan dan mengkilap. Memiliki suara yang halus dan khas: tersusun dari dua nada.
Ayam bekisar, karena ia hasil persilangan antara dua jenis yang berbeda, biasanya mandul. Namun, tidak semuanya demikian. Ada pula ayam bekisar (jantan atau betina) yang bila dikawinkan dengan ayam kampung menghasilkan keturunan.
Ciri-ciri khusus dari ayam bekisar yang paling menonjol adalah bentuk bulu leher yang ujungnya bulat/lonjong bukan lancip. Jika dibandingkan dengan ayam jago biasa maka akan terlihat jelas. Bentuk ayam yang mirip sekali dengan bekisar adalah hasil silangan ayam bekisar dengan ayam kampung yang dinamakan bekikuk. Bentuk dan posturnya sama, hanya kadang-kadang pial dan bulu lehernya yang berbeda.
Tahukah anda ternyata di Sentolo Kulon Progo DIY ada seorang penangkar bekisar yang cukup handal, ibarat seorang maestro seni. ang mengatakan bahwa menangkar bekisar itu ibarat membuat karya seni.
“Memproduksinya ibarat membuat karya seni, sementara pembelinya terus mengantri”
Jaringan pembibit, pedagang dan komunitas penggemar ayam bekisar tersebar di seantero negeri. Seandainya bibit ayam bekisar diproduksi dalam jumlah banyak pun dipastikan akan tetap laku.
Walaupun demikian, persis seperti barang seni, tak mudah untuk membuat karya dalam hal ini bibit ayam bekisar yang masuk kategorimasterpiece (karya besar). Lebih dari itu, meski pembibit membatasi jumlah produksi, biaya produksi tetap rendah. “Khas seperti barang seni. Antara harga jual dengan ongkos pembuatan sangat jauh dan sangat untung. Tapi sayangnya tidak semua orang bisa membuatnya,” ungkap Agustinus Ariyanto, pembibit ayam bekisar asal Sentolo, Kulon Progo, Jogjakarta ini.
Menurut peternak yang akrab disapa Agus ini, di komunitas bekisar antara peternak dan penghobi garis pembatasnya cukup jelas. Peternak seperti dia kebanyakan hanya berkonsentrasi memproduksi bibit saja, untuk dijual. “Meski nanti bibit dipelihara oleh penghobi, kemudian memenangkan kontes sehingga harganya melambung hingga setara harga mobil mewah, kami ikut senang karena pemenang itu bibitnya dari kita,” paparnya bersemangat.
Sebaliknya, penghobi jarang yang bisa beternak (pembibitan bekisar). Sebab ayam bekisar adalah final stock persilangan yang mandul, tidak bisa dikembangbiakkan lagi.
Produksi
Setiap 5 hari, Agus mengaku mampu mengeluarkan 80 – 100 ekorkuthuk (DOC) bekisar. Perbandingan jantan dan betinayang dijual sebesar 50 : 50. Pejantan dijual Rp 60 ribuper ekor sedangkan betina Rp 6 ribu per ekor.
Alumni Fakultas Ekonomi Universitas Pembangunan Nasional Jogjakarta ini mengungkapkan harga kuthuk tak pernah turun. Padahal, menurut dia hanya bekisar pejantan yang ‘berfungsi’ atau dikonteskan. Karena kontes ayam bekisar meliputi warna bulu, warna kaki, postur, kebersihan, dan suara kokok. Selain itu, ayam bekisar betina tidak produktif (kalaupun bertelur, fertilitas telur sangat rendah) dan berdaging tipis.
Maka Agus berpesan kepada penghobi bekisar pemula untuk membeli bibit ayam bekisar di tempat yang jelas. Bisa ke pembibit langsung atau kepada sesama penghobi agar tidak tergiur harga miring tetapi ternyata yang didapatkan adalah kuthuk betina.
Untuk memproduksi kuthuk bekisar, Agus menyediakan pejantan ayam hutan sebanyak 30 ekor dan 200 ekor ayam kampung betina. “Pejantan harus ayam hutan hijau, asal dari Jawa, Bali, dan Nusa Tenggara. Kalau induk betina paling bagus memakai ayam kampung dengan warna bulu wido,” paparnya. Ciri wido adalah bulu berwarna seperti klobot (sarung tongkol jagung) dengan variasi kuning. Ayam hutan dari Sumatera dan Kalimantan tidak dapat dipakai karena berjenis ayam hutan merah.
Pejantan ayam hutan yang dipakai Agus adalah hasil tetasan, bukan ayam hutan tangkapan. “Ayam hutan tetasan mentalnya bagus, karena tidak sempat hidup ‘liar’ dan tidak mengalami perubahan habitat,” ungkapnya.
Agus menyatakan harga calon pejantan ayam hutan hasil tetasan yang layak untuk pemacek mencapai Rp 750 ribu – Rp 1 juta per ekor. Bahkan pejantan yang sudah terbukti mampu mengawini dan subur bisa mencapai Rp 3 juta per ekor.
Agus memilih menggunakan kawin alami sistem dodokan ketimbang sistem IB (inseminasi buatan). Kuthuk bekisar hasil kawin IB biasanya kalah kuat dibanding anak dari kawin alam sehingga harus dirawat lebih teliti terutama pada sebulan pertama kehidupannya.
Sistem dodokan adalah pejantan dibawa ke kandang baterai betina, betina didudukan kemudian keduanya dipasangkan sehingga terjadi perkawinan secara akurat.
Kendala
Hingga saat ini, kendala bisnis pembibitan bekisar tidak berada di pemasaran, tetapi murni di teknis budidaya. Kendala yang umumnya di alami pembibit bekisar adalah kemandulan dengan gejala telur tidak fertil sejak 5 tahun lalu. Saat parah-parahnya kasus ini, telur tetas yang fertil pada candling pertama hanya sekitar 2 – 5 %. “Semua peternak pembibit mengalami. Namun dengan berbagai percobaan yang menghabiskan uang puluhan juta, berangsur-angsur bisa teratasi,” ungkapnya.
Sayangnya Agus menolak merinci cara mengatasi kemandulan itu. Ia hanya menyampaikan, dilakukan perbaikan budidaya, mencukupi nutrisi, mencegah penyakit dan menambah sistem kekebalan dan kesehatan ayam. Ia bahkan menghindari air sumur karena banyak mengandung E coli. “Saya pakai air dari perusahaan air minum,” tegasnya.

Its courtyard is a location for a Hindu statuary and artefacts collected including Balinese carvings. Apart from these function of being home to such collections, this museum is also a venue for wayang kulit performances.

Handycraft Villages
The Sentolo District of Kulonprogo Regency, has a handycraft villages like Sentolo, Salamrejo, Tuksono and Sukoreno, etc. each has an individual style of handycrafts and due to this individuality and uniqueness, the handycrafts coming from these regions are not only different but above par excellence as well.
