One of the biggest reasons why software project delivery dates and costs tend to be unpredictable is because bugs (defects) tend to be unpredictable. How do you predict how many bugs a feature will have, much less the time to find and fix the bugs, and how do you know how many additional bugs the fix will introduce?

The solution here, of course, is to have a very high level of engineering practice - Test-Driven Development, Continuous Integration, Object-Oriented Design, Refactoring... To many people, even developers, many of these practices have too high an upfront effort and learning curve, but the alternative is to never really have any confidence in deadlines or budgets.
