Rajeev - Take a look at this diagram of the Lync architecture [ http://msdn.microsoft.com/EN-US/library/office/dn466124(v=office.15).aspx ].  The application you create with VoiceModel will be deployed on a web server, which is depicted as item 7 in this diagram.  You need to point the voice browser (shown in green) to where the web server is deployed.  In Lync the voice browser is located in the host .NET application [ http://msdn.microsoft.com/EN-US/library/office/dn466127(v=office.15).aspx ].  At the bottom of the page describing the host .NET application is a link to sample code for VoiceXML.  Hope this helps.
