This patient presents with abdominial distension.

Q1. What is immediate management?
A1. History and examination. Blood profile for FBC, renal profile. Likely to need admission and catheterisation. Given bladder volume this may be best with a suprapubic catheter. Then careful observation for diuresis. Daily weights. Strict input/ output charts.

Q2. What do these images show?
A2. This shows huge fluid filled mass arising from the pelvis. There appears to be bilateral hydronephrosis. This is huge chronic urinary retention (4 litres) with hydronephrosis.
