What are some ways that you stay focused when starting a writing session?

When using dictation in the car, it's important to have a clear, specific goal for each session, otherwise you'll spend the entire drive listening to music, thinking about cheese, and doing other non-productive things.

What do you do when you run out of words before you run out of writing time? When dictating in the car, it's easy to find yourself driving in silence, unable to think of what to say next, or where to go next in the story. Does anyone have any tricks they use to keep themselves on track so they don't waste an entire writing session?
