What are the most common false positives when people think they're penalised?
http://dejanseo.com.au/false-alarms-google-penalty/
1. Blocking search engines in robots.txt / meta
2. Website redesign, changed URL structure, no 301
3. Server down or unreliable and slow
4. Authentication required to see the site
5. Running two sites with same content
6. Slight drop due to normal fluctuations or loss of a few links
7. Change in Google's algorithm
8. Poorly executed canonicalisation or URL parameters
9. Set the wrong country in Google Webmaster Tools
10. ?

Autocrat's already nominated robots meta and you've already listed Disabled Indexation but I think it's worth adding that most of them get that way by accidentally checking the "private" setting in WP.Jul 25, 2012
Might as well as

In my experience, most clients who see a rankings drop and think they have been penalized usually think it is a "mistake" Google has made. Issues with their own site, incoming links (devalued or outright spam penalized), and any of the hundreds of reasons rankings can drop rarely enter their minds.
The number one false positive I encounter is link devaluation.
The site has lots of lower quality links, like directories, and many of them get hit by penguin or panda causing the client's site to drop some too. Not penalized directly, but definitely not doing as well as it was.Aug 20, 2012
Forgetting to add back the Analytics code. FTW. :)Aug 21, 2012
