Another success story with person-centered supports using a discovery process:

"Andrew is enjoying his job at Hy-Vee and a variety of responsibilities. First and foremost, he's a grocer and bagging groceries. But he also stocks shelves and does some general cleaning. He brings experience in the business with him but his previous job was very limiting and not the best match, where he was retrieving carts and didn't like the outdoor work. Here, he's indoors, likes stocking shelves, likes the variety, and much prefers the Hy-Vee culture.”

"Putting Students & Families in the Center of our Planning" session with Sue Benolken from MDE and Amber Maki from DHS is one of the many great sessions scheduled for Thurs, Nov 8th at the Minnesota School Mental Health Conference in Brainerd.

Description below:
"Person Centered and interagency coordination practices are valuable tools to enhance learning experiences and mental health well-being. This presentation will provide context as to the concepts and requirements of interagency and person centered practices, the value of these practices to school and mental health populations, and the partnerships across agencies to implement. Real world examples of work being done in these areas will be shared."

Person-centered supports from Mounds View transition program using a discovery process for career planning.

“Emily has recently changed jobs and is now working as a nanny. Her love is working with children and she is definitely gaining a great deal of experience in the field. Emily has worked in day care centers and has had several previous jobs as a nanny. In addition to her regular 20 hours per week, she has made herself available on call for additional hours.”
