In my games (both ones I've GM'd and just been a player in), we don't really escalate much because no one wants to die from a random bullet. Our violence winds up being stealthy, to avoid social consequences, and so brutal that immediate reprisal is nearly impossible. However, if it was an area where we weren't going to stay long, we'd be far more liberal about the whole thing.

Most of the games I've played in give XP for sessions or story arc completion, which gets rid of the minutia of XP management, or the ridiculousness of killing so many chickens you become a combat god.

Communications and sighting were also important, as was training. Training and communications were talked about a lot, but I remember sighting being one of the oft unheard of things about tank design. Being able to see your opponent is the first step to engaging and destroying them in many cases. The Soviets (who famously had poor training) also had very poor sighting tools, requiring them to pop the hatch in combat or hunker down and let their armour go to work.
