Not Sure How Your #Pharmacist  Can Help? Start here.
Is there anything I can do to help increase the benefits and decrease the risks with my medications? Are there generic versions I could take? Can I stop using the medicine or use less if I feel better? (The answer is usually no!) What should I do if I take a dose incorrectly or miss a dose?

#patientcentered  care:
Video Ever tried to get sleep in a hospital? Ever wonder if anyone even taught them what care is? Some hospitals are great, but some sure aren't. Fixing that is the idea behind patient-centered care, which the Institute of Medicine calls "care that is respectful of and responsive to individual patient preferences, needs, and [...]

Merck Recalls Cholesterol Drug Liptruzet
Packaging defects have prompted a recall of a combination #cholesterol drug called #Liptruzet produced by Merck & Co., temporarily affecting the entire U.S. stock.

#Merck said the foil pouches holding Liptruzet pills could allow air and moisture inside, potentially decreasing the drug's effectiveness.

The recall covers all four dose strengths and every batch that's gone out since Liptruzet hit the market in May.

Liptruzet combines two medications that work together to reduce "bad" cholesterol levels:
#Atorvastatin the generic version of the widely used cholesterol medication #Lipitor, which reduces the body's natural cholesterol production, and
#Zetia (ezetimibe), a Merck-produced drug that decreases the amount of cholesterol absorbed from food.

Cardiologists expect no health problems to result from the recall, in part because both atorvastatin and Zetia will remain available separately.

Merck said it plans to get Liptruzet back on the market as soon as possible.

Decreasing Healthcare Costs While Improving the Lives of Patients
Introduction
The Pharmacy Home Project works alongside its Partners and Collaborators to promote medication optimization through the creation of Pharmacy Homes.

Mission
The Pharmacy Home Project’s main goal is to promote and encourage medication optimization programs across the spectrum of care that lead to well-coordinated, goal-oriented, continually reinforced drug use plans. Collaboration between pharmacy and other healthcare settings provides comprehensive medication management by including the patient and the patient’s goals in the care plan from one setting to the next. The Pharmacy Home Project includes a well-coordinated network of pharmacists, nurses, physicians and other healthcare professionals who share responsibility for patient goals and work to improve the quality of care while decreasing overall healthcare costs.
