I think many of your "wishes" could be achieved if doctors didn't depend on insurance companies. It is like the old saying, don't let the guy with the broom decide how many elephants to put in a parade.

Doctors want to provide a better service. Patients desperately want better service. But it isn't in the insurance company's best interest to accomodate the needs of patients and doctors.

In my world, patients would pay the doctor directly for their services, just like any other professional (eg, attorneys and accountants), and only have insurance for catastrophic events. Much like we buy insurance for our cars.

Until we are able to shift this backwards systems where the guy with the broom decide how much a doctor's services are worth, we will continue to have a mediocre healthcare care system that benefits the insurance company before patients and doctors.

Got a note in my inbox yesterday from a payer alerting me a patient (w a BMI over 95%) needed labs. Went to the chart, saw I ordered the test (a fasting lipid panel) Dec 2011, but family never got lab done. Called patient to return. No loop in EPIC to alert me that this family didn't do labs. It was the payer that closed the loop here...
