Ok... so I'm never sure of the proper etiquette with this stuff. I hate to seem like I am complaining or outing someone.  I really would just love some opinions on some stuff I'm seeing rank and can't figure out why. 

First site: 
texasmedicalweightlossclinic (dot) com is a local weight loss clinic that is ranking in the top position for a few national keywords. Their link profile is one of the worst I have ever seen yet according to SEMRush they have been steadily increasing for more than a year and currently receive $10,860 worth of monthly organic traffic.  

Second site: 
buy-otc (dot) com has been gaining market share for the past year but I'm unable to find a single quality link or reasonable explanation for their success.   They seem to be capturing at least $2,987 worth of organic traffic with less than a handful of links. 

This is a space I am in and while it's not overly saturated, it's not so niche that I would expect to find stuff like this.  I'm wondering if anyone can find a redirect I'm missing or some other trick these sites have employed. If not can we then assume that there are other signals, not yet widely discussed, that are replacing links as the most powerful ranking factor?  

Anyway... sorry if this is the wrong place for this.. I know that there is always some engagement in this group and I would love a second opinion.

Has anyone seen an instance where Pigeon impacted non-local organic search?  I have an issue that seems to coincide with  Pigeon but the site is not a "local" website. 

I have a client with an informational website that targets long tail phrases like "how to report an online scam".  They were blown out of the top 100 for almost all of their keywords around the time of Pigeon and we're trying to figure out why. 

If it is a penalty, it's a severe one.  Unfortunately there is no message in  webmaster  tools and it doesn't line up with any major algo updates (other than Pigeon).

We've checked all of the technical issues and it seems that the site is being indexed and seen by Google without  issues.  I'm also confident that the content is well written and original. 

There are a few questionable incoming links, but nothing I'd normally worry about.  

Any ideas that people might have would be greatly appreciated.
