Oregon Health & Science University (OHSU) launched an effort called "Caring Wisely" that focuses on patient's Palliative Care throughout their journey, not just in the final stages of illness. One effort of Caring Wisely encourages patients to provide Advance Directive end-of-life wishes before the big decision need to be made. Read more about his effort based on the #ChoosingWisely palliative care recommendation.
