Vignette: Renal

A Malaysian 6-year-old girl is brought to the clinic by her mother because of diarrhea for 3 weeks. She appears listless and confused. Vitals reveal HR: 160, BP: 80/40 mm Hg. Physical examination reveals facial swelling, acieties, and 3+Pedal Edema. Serum albumin concentration is 1.5 g/dL.

What is the most common cause of the patient’s syncope if not caused by cardiac origin in this age group?

Vignette:

A 42-y.o. female is admitted to the hospital for bouts of syncope and dyspnea. 10 years ago she had undergone mitral valve replacement. X-ray shows an enlarged heart with Kerly B lines. Echocardiogram revealed left atrial pressure is greater than the left ventricular pressure toward the end of diastole. Course crackles are heard in the lower lung field.

A 26 -year-old IV drug abuser presents with dyspnea & a murmur. The murmur is a holosystolic, medium pitch blowing at the apex radiating to the midclavicular line, S3 & JVD are noted on examination. BP: 110/60 HR: 115 irregular Temp: 104F (40C). Course crackles at the base noted on lungs auscultation.
