Beyond a Survey: Use Health Risk Assessment (HRA) tools for behavior change. HRAs can open the door to health awareness & a one-on-one information exchange with the participant. Each HRA question can become a decision point for the participant to reflect on their current lifestyle choices & possibly consider lifestyle changes, making an HRA a critical tool in any health & wellness program. http://www.lightenup4lifeusa.com

Diagnosing a community means evaluating the overall health of the community: CHNA, lifestyle choices, physical & emotional wellness. After identifying the issues, decide on a strategy that will reach the most, offer a variety of activities which will maximize participation, and measure outcomes. http://blog.lightenup4lifeusa.com/

Brilliant and so true...
The prevailing sentiment in pop culture nutrition - propagated in books, blogs, and blather; documentaries, and diatribes - is that everything we thought we knew about diet and health until yesterday

LightenUp4Life™ Online Population Health Management Program That Engages Your Patient Population and Increases ROI
Introduction
LightenUp4LifeUSA.com  is a web-based wellness program for hospitals to use in collaboration with their community and local employers, LightenUp4Life™ has a six year proven track record for lowering obesity rates and increasing overall fitness, tracking overall participant health metrics, increasing employer and patient interaction with their local hospital, increasing patient volume, and supporting other healthcare marketing and branding endeavors.

Population Health Management and LightenUp4Life™

In considering the long-term health of U.S. citizens, the Affordable Care Act’s intent is to solve two broad issues. First, control the unsustainable rise in healthcare costs. Second, the rise in healthcare costs cannot be contained without improving the overall health of the U.S. population. Hospitals must reach out to the populations they serve making an effort to manage their population’s health and well-being. Hospitals that use LightenUp4Life™ have addressed health management through integrated wellness programs and community building to engage and interact with the populations they serve. Success in the News: Read how Mission Health and Ingles Supermarket used LightenUp4Life™ to fight obesity in WNC.
