The Conversation Project has excellent free resources for advance care planning, with a focus on providing tools for people to talk with their families and to talk with their providers about their end-of-life care preferences.

I've found that the Starter Kit is a great tool to guide the person through what they should consider and discuss with their families before they complete a legal advance directive. I've used and seen it used effectively in the following ways:

As a handout at events promoting advance care planning, that people can do themselves or give to others.

As :homework" for patients and families to do before a goals of care / advance care planning discussion with providers.

As a conversation guide to help nursing and other healthcare students as they are learning to talk about about end-of-life care preferences with older adults or people with serious illness.

This site also has a great resource for "Talking with your doctor:, empowering patients to begin the conversation rather than waiting (sometimes in vain) for their provider.

Community outreach event tools are also available, so you can organize and conduct your own advance care planning events. They will also allow you to use your organization's name and logo on their materials.
