I always find it amazing how we were created to adapt to degenerative changes. I had a patient yesterday that was wrought with severe arthritis in her TM joint, and yet her only complaint was "morning headaches." Then the patient with perfect anatomical joints in severe TM pain! I know one could argue that pain does not come from the bone, but it would be impossible to have the bone melt away w/o some soft tissue changes! I think it is about adaptation. One patient is adapting to degenerative changes and other is not. Risk factors drive degeneration and we are called to intervene when the patient can no longer adapt (pain or loss of function). That is the way I see things at this point in my journey to heal those in facial pain.

Just submitted my abstract on the Custom Face Mask to the dental sleep academy (AADSM) for publication and presentation at the next national sleep meeting next June in Boston. Dr Jerry Jerald H Simmons is my co-author. Hope it gets accepted. I have been able to submit an abstract every year for the last 8 years now. A great way to stay on the leading edge of our profession. I suggest more sleep dentists get in the habit of doing this. The profession needs more dentists doing research and observation. Both the profession and more important the patient...wins!

Ever had one of those patients that the symptoms presented do not really match up with the pathology you see in x-ray and exam? Had one today. All signs of significant clenching: small airway, joint tenderness, muscle tenderness, high palate with torus and HA. She denies clenching stating that she sleeps with mouth open. only symptom is "pressure in ear and ear ringing." Hmmmm. The challenge here is to relate the two so that she allows me to treat her. Love this job!!!!!!

3 out of 4 new patients yesterday had significant TM joint arthritis. Interesting thing is that joint pain was NOT their main complaint. Yes it was tender to palpation, but their main complaint was muscle related pain or contralateral side joint pain. People will dismiss this as saying that everything is "compensating" for the arthritis, but from a medical science point of view, I wonder what is really going on. Yes, in each case there are anatomical changes in the jaw, but I wonder if complex supporting jaw muscles are just trying to avoid parafunctional forces on an "injured" joint. Like all things with our body, just trying to adapt to degenerative changes!!
