Another success at Pattaya Imagration yestarday got my Visa with the same docs as last year. I was told by others that I would need a rental lease contract this time but that didn't end up being the case. Just my True Internet service bill that was dated and had my address on it was all they needed to prove I had an ongoing contract at that address with my name on it.

4 day stay at Siracha Hospital.
It wasn't really a planed thing. I just went down as planed with Mick as my driver on a follow up from my 10 day ago visit to siracha Hospital. I expected to give them the status of my Knees (right and left) and have them prescribe me more drugs. Well it didn't quite turn out that way. As the first thing they did when you got there was to give me another blood test. I guess part of the plan to see how there drugs were acting. So they finally called my name after what I assume is the typical long wait of about 3 hours this time (only half the time of my first visit that ended up being 6 hours). My time came and I was called into a small office with a nice looking young lady doctor behind a computer terminal. She started asking me much of the same questions they ask me the first time. And I repeated off the time frames of each of my conditions about the same as I told it the first and second times. She also had the blood test results on her terminal and I don't really know what she saw there or in the story that I told her and to note my knees hadn't really improved at all at this stage. In fact I had just been recovering from a major re-inflammation in my right knee that had blown up like a balloon just 2 days before while on there present prescription, but now the swelling was more about the same as when I came in for my first follow up 10 days ago. She said she needed to call some one for some consulting about this. She was on the phone with someone for a few minutes speaking in Thai that was far above my level of understanding in most the subject mater. After she got off the phone she says that they would like me to check in so that can take a close look at my condition. She asked “would it be OK for you to check in today?” I was a bit stunned as it wasn't anything I was really expecting but I thought sure lets fix this shit. And I accepted her proposal to check in for no real known period of time. She gave me a vague sense it might be one or two day stay, but I expected it would be more like a single day thing for sure. What more can they do that can't be done in a day I thought for something as simple as this? So I accepted her offer. So from that response she said they are also asking for more blood samples. I had just given them a single pull of blood just hours ago why would they need more I thought but said nothing other than “ Sure take as much as you want”. So they immediately wheeled me into another office were I had my blood pulled a few hours ago. But this time they wanted two pulls of blood and one from each arm. I guess they took me serious when I said take as much as you want? Any way no problem they are very good at it and no real pain with this expert blood pullers. Next I was wheeled up to the 4th floor ward where I was checked in and provided a bed that little did I know would become my home for the next few nights. The room I expect was one of there better ones as it was the only one that I saw on the floor that had any real walls. I think it might have also been the only room that had any air-conditioning, but later I noted that the air-conditioner wasn't running and the windows were wide open. The room was situated with 5 beds in it that were all full but the one they had for me when I first arrived. On the bed next to me on the right side there was a sick Buddha munk that still whore his red outfit even in his sick bed. All the others In there beds had some light blue uniform that must be the standard issue clothing provided by the hospital. I got myself situated and found an electric plug to plug in my android phone that I knew was going to be my only source of entertainment here and I had played with it a lot in the day as there was noting else to do when waiting and now the batt was down to only 12% level by this time. There was a small 13 inch TV in about the center of the room that was then playing some Thai soap opera of something like that, that I was sure was not going to be much fun for me to be watching for any amount of time now or in the future. Shortly After my arrival they had a short meet and greats with some of the hospital staff in that section. When that completed I thought it best I just took a bit of a nap. But it didn't seem long that an army of doctors came into the room I think I counted about 6 of them. They gathered in front of my bed with one I noted was some kind of lead doctor or teacher that was explaining things to the group I noted were all younger and expected must be ether interns or students. Then the teacher as I call him now started asking me more or less the same question mostly focused on a time line of how long had the present inflammations been in existence. A few of the students as I called them would also break in and ask other good questions including my parents history of anything similar and other such questions that I can't fully remember as there were so many questions. After all the questions the teach doctor says “ya it sounds like an infection. I'm going to give you an anti-biotic solution drip”. I replied OK but I really don't think it's an infection, I think it's more likely some kind of auto-immune problem. He shrugged and OK lets go. And that was the last I saw of most of that group. At some point I asked one of the nurses is it OK if I take some of the anti-inflammatory that I had brought with me. I hadn't taken any meds since the early morning now as I was awaiting the new prescriptions that I was expecting from them that they had noted also in the group that they were going to provide anti- inflammatory and or pain killers. She said NO don't be taking anything you bring, only what they now prescribe. I agreed with that. They need to know what all I'm taking to prevent any conflicts. So I refrained from any of my drugs thought out my stay that was now only 3 ibprofin left in my stash anyway. They later turned down the lights and the place got mostly quiet other than out in the hall I could hear one man screaming about something (probably in pain) but it was far enuf away not to bother me too much. My problem now was that those drugs never did turn up other than the anti-biotic that they now had plugged in and dripping into my wrist. So my pain in both legs now started getting worse. Now it no longer required moving the knees they would both throb with pain just a bit more in the left side than the right but they both hurt. I almost got 2 hours of sleep from about 12:00am to about 2:00am but by 3:00am the pain was getting maxed out. I kept trying to move and exercise them a bit and try to find some way to get comfortable. I forgot to mention I did ask for a second pillow to the nurse when she was installing my anti-biotic needles, but her return was oh we are all out of them. So now besides only having a single pillow the bed was more like a plywood plank with just less than half an inch of some rally hard closed cell fome on it. I guess this is the type of bed most Thai people sleep on as I see in some girls rooms I've been to they just sleep on a cement floor with a straw mat, so to them this is still deluxe. But to me I'm used to a standard 8 inch matrices with box springs that I normally sleep on. This bed for me was hard to lay on any side for much more than an hour at a time with out something going numb or getting sore. So I had to sacrifice what pillow I had to my knees and made the blanket into something that would work for my head. This is the best we could do with the material available for at least this night. And pain wasn't the only thing to help keep me up after 3:00am I was getting hungry. I really had almost nothing to eat the day before just a small tuna sandwich I got Mick to get me before I got him to go home. But lucky this hospital seems to have breakfast early like around 6:00am just after the 5:30am blood pressure and temp testing time. That put a smile on my face until I saw what was in it. It was just a single bowl of rice porridge with two small fish with no heads but all there rest of them still there. But I was so hungry I still ate it and almost liked it, not that I would EVER buy one if I saw it for sale any place in the future. I started thinking this might be the best food I will ever see in this place so I best start getting used to it. I didn't see any of the other prisoners I mean patients complaining about it in the room. They all seemed to be happy with what they got. But maybe they had a different cook that made what they got? I didn't complain about anything to anyone and took it all like a trooper. But my pain was getting steadily worse and it wasn't for some time before I saw my new set of doctors on the sean at around 9:00am. This time it was only 3 doctors, two younger girls and a young guy. The guy seemed to be more of the leader and the fat girl also had much input with the third girls was mostly quite but very cute. Again almost the same questions looking for a time line for the pain and suffering and it's location. As I didn't hear any ideas coming from any of them as to what they though it might be or what they planed to do next, I felt I best throw some of my theories and test that could be done to prove them wrong. First I suggested was the more accurate rheumatoid arthritis blood test. A quick return on that from the mostly quite girl said “but that type of arthritis only acts on smaller joints like the hands”. I wasn't sure that was correct but it wasn't wrong and I return “well you got me there”. And went on with my next choice of test that off the top of my head was a blood test to see if your reactive to an allergy that is something like an IGG test. They looked at me as if they never heard of such a test. I had hoped they already knew it so they could correct my inaccuracies in the name so I didn't have to hunt through my notes to find it but could tell I was going to have to provide them the exact id of the test to have any hope to have it. So I started looking in my phone that I had put every note I had from my computer on it. But this was a new phone due to my last being stolen only weeks ago and it was taking me some time to figure out how to use it as well as find the needed document that I put the note in. it was taking much too much time for them to wait and they said OK we got to do our rounds. I said OK but don't forget to at least add some anti-inflammatory drugs that you seem to have forgotten last night. I explained that I only got about 2 hours sleep and suffered and was suffering in pain and also noted that my left foot had now also swollen up more than it had been the day before maybe due to note having any anti-inflammatory drugs. They said oh don't worry we will hook you up. And they were gone. The clock ticked and the pain grew and lunch came. OK things were finally looking up. This time the food wasn't that bad, more my standard Thai style with rice and some cooked veggies with some canned fish with tomato sauce and a light soap with some sliced port and some radish. Not bad. But still no drugs. Click click the clock with more pain and boredom then 4:00pm comes along and it's dinner time. Again some good Thai food much like the last one aways with some fish and sometimes pork. Shortly after dinner I think I got my first drugs ever. I'm sure one of ( don't remember if there were two drugs or not) was an anti-inflammatory (Naproxen?) but I'm not sure. What ever it was it just didn't seem to kick in as it seemed I was still in pain and seemed to continue throughout the night. Maybe it takes time to build up a dose in your system. From what I originally had in me at home I was taking 4 doses a day not just 1 pill over 2 days. so any way we continued to suffer this night in pain. Morning came and much more of the same repeats with some good food and pain and boredom. Even after they collected another sample of fluid this time from my left ankle that they would later find no crystals of any kind and this time I looked myself in the microscope at the sample that they were happy to show me. I verified there was no crystals to be seen on the slide of my fluid that they had setup presented to me. They included what a sample with crystals looks like to give me a better idea. Now with a two to one vote there new sample and the sample from BKK hospital that I showed them the photo of the report also showed no crystals of any kind. There diagnosis still stood that it was gout and planed the same regiment of drugs that I had already been taking now for about 17 days now with no results. I just shrugged and said well in all reality at this point even if I do have a auto-immune problem it too would be treated in much the same way so I didn't argue with the young male doctor about it at this time. I had one last meeting with the full class and the big and I feel best doctor teacher #2 that they had so far that had been working with me (the one that was able to pull the sample that the student failed to pull with only hitting my bone 2 or 3 times). At this point the teacher#2 just re-said about the same thing that the young doctor told me a bit earlier. And stated the list of drugs they planed to prescribe me. I returned with “ you do know that is the same prescription I've now been taking for the last 17 days don't you?” I added, “maybe up the dosage”. The teacher said “I just did”. My eyes lit up a bit and I asked him “ oh by how much? “ he said “to 30mg per day”. I had to think about this and wasn't sure off the top of my head, what I was already been taking in the past, I needed also needed to look up what it was that bonglamung clinic had originally prescribed and to find out what the long term effects of such a dosage would be and just how long can you continue to be able to take it. But at this point is sounded like a good idea to me with what data we had in hand. So I gave him a some what positive response. His next statement also was a bit of a surprise he asked me “do you want to go home”. It didn't take me long to give him a response of yes and we negotiated at what time they would expect a check-in to continue to monitor and adjust the dosages as needed. I tried to put in that I was open for some remote (like over the phone) monitoring and action taken but it seems that is just never done here as I got no response but unhappy faces. So that sounded final to me that I was going home. but a few minutes later The fat girl doctor returned and tried to put in a few points that she felt would be to my benefit to stay that apparently she didn't feel comfortable speaking about it in front of the teacher#2 ( her boss?). But to me it just didn't add up and I had gambled and spent too much money, time, pain and suffering on this project as it is for what little it seems I had gotten so far in return. If they had any other resource they were willing to offer they shouldn't have waited 4 days to start making any use of them. I went a head shortly after this last meeting and contacted Mick to see about him picking me up at about 3:00pm if possible. I knew he had a conflicting run to do on this day and was going to see if I could some how stack in and get in on his return from his airport run. And he said he was already planning for it as a backup and had already packed my wheelchair in his truck. So I was all set with transportation out of there and looking forward to my departure. I again Asked several of the staff I could find that would speak to me 2 more times 2 hours before then 1 hour before my departure to provide me the lab results that I had been asking for from day 1. Each time they said “don't worry you'll get them”. I said from where? from who? with the departure documents and bills? I took her response as a yes. And as far as I could see there was nothing more I could do about it. When Mick was about 20 minutes out as I could see on the GPS tracker, I went and started asking what I needed to do to check out, my ride was only minutes out I wanted all the paper work completed and bills paid and just jump in the truck and split. She said you need Mick to do that. What? What the fuck did I sign when I put Mick's name on the sheet of paper when I checked into this place. It was in Thai and I'm clueless what it said. I'm not sure what status I promoted Mick to but it seems they had me by the balls without his signature. So Mick came in as expected but now had to go find parking that also took more time, and now we had to do all the paper work after he arrived that takes about 30 minutes or so that should have all been taken cared of before his arrival. But hell I was still happy as a clam just to be out of there. He even brought Ann my number one girl with him to cheer me up. And she seemed happy to see me and had fun pushing me around in the wheelchair as she is like a playful child one of things I like about her. As Mick went to get his truck out of the parking structure. Me and Ann worked on figuring out how I could access one of the ATM machines that are not at all wheelchair friendly. I was able to get a guard to give me a hand over the big ditch between the ATM and the street. I finally got my transaction and needed funds to pay off my remaining debt to Mick for his ride. Mick truck then arrived, We all jumped into the truck and we were out of there. The End.

From the results of this test from the hospital today seems to conclude that I have pseudo-gout.  It shows from my joint fluid sample that I have Calcium Pyrophosphate Dihydrate that is a sign of pseudo-gout..  standard gout would have uric acid crystals not Calcium.  it's the opposite of what I had been treating myself for  as I thought I had standard gout and had been taking alloprolanol that would I guess make it worse.  But I don't understand why the BKK hospital tests that ran the same test on the left leg did not see this same result.  also my uric acid levels are just on the edge of high 7.0 that you would expect to be what someone that get's standard gout would have.  So I'm NOT fully convinced but I'm going with this treatment for now to see how it goes.  To see the entire set of blood test results and bills and other documents from the visit to Siracha Hospital, see the google drive link:

A day at Somjed Sriracha hospital
On July 22, 2016  I got Mick to take me to the Somjed Sriracha hospital to have them take a look at my swollen right knee.   I now had both my knees swollen with pain when I would put weight on them and could barely even move with a pair of crutches at this point.  I managed to make it down to the lobby of PBC and to the front entrance to meet Mick with his truck to pick me up.   At the hospital I was able to barrow a wheelchair to continue the operation.  I was able to find some one to help me sign up in there system as I had never been to this hospital before.  After that they wheeled me directly to orthopedic department that looked packed wall to wall with people.  The nurse said it would be about 1.5 hour wait as I expected.   About 1.5 hour later they called my name and I called back in response.  The nurse looked over and saw me and waved just a moment.   That moment became about another 2 hours or so.  Then the nurse came over and offered me some water and said the doctor had me setup in the Q as number 3.  about 30 or so minutes passed that the nurse came and wheeled me into what looked to be a very small  operating room with no bigger than about 8 X 10 meter that they were doing surgery or something on 4 different people and 2 ready to be worked on people and myself also stuck there.  This room also appeared to be a hallway to move to 2 other parts of the complex that people and staff were moving through it to get to other parts.  As I saw an open leg that had 4 inches of open flesh showing with no curtains,  I would also see 4 people walk through the room.   After being stuck in this room for about 20 minutes with nothing done to me they finally took me out of this room and back into the central waiting room again to wait another 3 hours or a total of 6 hours before I was again wheeled into that operating room and then shortly later taken to an office with a very young doctor.  He looked to be about 25 years old or younger so I assume maybe fresh out of school.  He spoke pretty good English and had a very good attitude and we started to get something done.  After he collected my recent history he said he thought we should drain the right leg and again analyze the fluid to try and isolate the problem.  He was still convinced from my history that it was gout.   To me it sounded like the only path as it seemed the leg required to be drained no mater what the cause at this point.   I was then wheeled back into that operating room again but now it had less people in it as most the people for the day had already been treated by now.  In about 10 minutes the young doctor arrived with some of the tools he would need to drain it.  I forgot to mention earlier that the doctor did ask if I would require any pain meds and pointed to his arm as in a shot of morphine as I suspect.  At that time I had turned him down saying no I'm not in much pain now, only when I put weight on it.  But little did I know what I think he meant is during and after the operation.  This was a much more painful procedure than what I just had  in the BKK hospital 20 days ago on my left knee.  And ended up continuing to be painful for the next 2 days.  After the operation they told me it would be about 1.5 hours to get my test results.  Mick my driver had now returned and hung out with me for the rest of the time we waited for my results.  2 hours went by and my young doctor arrived and told me the bad news that they couldn't perform the test as the lab closed at 4:00pm.  I would have to return the next day to get the results.  He also explained they he couldn't prescribe anything until after he got the results of the tests.  I said OK whatever.  They set me up with my paper work to go and pay my bill that ended up being only 700 baht.   So me and Mick were out of there and headed back home to pattaya.  When we got to my PBC condo the pain in my Right knee had increased and my left Knee was still bad but now was better off than the right.  I was barely able to get back to my room with a pair of crutches with some help of the security guard.   When I got to my room the pain in my right knee was throbbing and continued into the next day.  Also by the next day the right knee had swollen up to even bigger size then ever even before the operation.  I had wondered why the doctor didn't prescribe at least an anti-inflammatory drug in post opp.  So I prescribed some to my self including Ibuprofen and a few hours later  diclofenac sodium (anti-inflammatory medicines (NSAIDs)) and continue with just  fenac every 8 hours after that.  The pain and swelling continued for 2 days but begun to break with the pain now down to only when I put weight on it again.   I couldn't even return to the hospital as I couldn't even get myself to micks truck.  I now have a wheelchair from Father Ray foundation (free by the way with delivery) so am now able to start getting around again and if needed get to the truck to get to the hospital.   So the plan now is to wait till Monday when the full results should be in  that include the culture of the fluid in the knee to see what might be in it.  As this hospital will not email or provide you the results over the phone I didn't want to have to make two 2 hour truck rides just to pick up the last result.  So I felt it best to wait it out and see.  So that's Went to bkk hospital today.  result is that my present knee problem and probably all my other joint problems is not gout but some kind of auto-immune problem.  they were able to drain the knee of 80cc of fluid.  felt like it poped when they stuck the needle in with so much pressure.  I also got some new drug Arcoxia that I've never tried (only 7 tab for 7 days), hope it works.

Pattaya Thailand Immigration for people from USA still working with the same documents as last year. Used USA embassy affidavit for income with my True Internet bill for proof of appartment location and a document from my bank showing I had an account. also a face photo and copies of pages of my passport. They seemed happy with that.
