61 yo WM with PMH of CAD with one previous stent many years ago presents after collapsing outside front door of ED. Pt is hypotensive and altered, but is able to communicate he is having chest pain and sob. Here is the first ECG followed by the second obtained 30 minutes later. Thoughts?

I don't think it is possible to tell whether it is AIVR or VT, but they are very different entities: AIVR is an automatic rhythm, and VT re-entrant.

Puzzling case. 42 yo male with h/o hypothyroidism presents with altered mental status. Complained of headache initially, then vomited several times. Completely normal vital signs. Physical exam shows he is uncooperative, but moving all extremities. Remainder of exam, including neuro, is normal. Head CT negative per radiology (and the neurosurgeon who was a family friend). WBC 13k, K 2.7. Remainder of labs negative, including TSH, alcohol and drug screen. ABG performed and I have never seen a more normal reading other than in text books. I intubated him initially just so I could do the above work-up. CXR negative. Lumbar puncture with 1 WBC, 3 RBC, normal glucose and protein, negative gram stain.

Any ideas?

Saw something similar a few years back in a young woman. Came in pretty unresponsive, was intubated. Head CT + MRI, LP and labs normal, nothing obvious in the first pseudo-EEG (used BIS for that). Was discharged as "non-convulsive status epi" under the assumption, that the RSI meds ended it.
