I Left the following thoughts…
I congratulate you on the need for, and lay-out of your very extensive program.
There are some differences in focus, and order, I would like to share.
#1. My preference is to place your # 9 "Are you willing to change your work-site culture?" to first decision (#1). If, as a leader this decision is not made early in the process, and appears an afterthought in any way, I would predict an early failure, and a waste of talent and resources.
#2. I think it is important to uncover the employees' point of view and desires. This takes into consideration Rogers' Rules of Engagement which considers the eagerness or otherwise of Employee participation.
I believe we are changing behaviors and cultures with work-site wellness programs, and it is not only to lower the cost of health care. The effectiveness, efficiency, productivity, and creativity (Human Capital Value) may have to be learned until they are the new normal, standard fallback behavior.
#3. From the 'physician' POV, which I cannot lose, I want to know more about the employee's health early, going into the program.
(i) "Where have you come from?" - what is your past experience with your own wellness?
(ii) "Where are you going?" - what do you want to achieve in 'our' wellness program?
(iii) "If you were ever well and fit, are you still well and fit? Or have you had a setback?
(iiii) "If the program you were in failed you, why and how? Why did you leave the program?What do you need to change in yourself or the program?
#4. I am a strong proponent of Measurement, and use a device which gives me markers to guage progress.

An extremely important point of view in the philosophy of "Healthier is Wealthier". Addressing and reversing autoimmune disease is important to the goal of increasing Human Capital Value, and reinforcing the mantra "Food is Fuel".
