Can anyone confirm/deny the following regarding implementation of hreflang tags for a multinational organization?

We are using the x-default tag to identify the US .com site as the default. Do we also need the hreflang=en default to .com, or is it assumed because of the x-default? The use case would be a South African visitor that spoke English for example. We don't have a site specific to SA, but assume that user would prefer UK english over US, and don't want to force them to US if Google would choose UK based on their personal settings.

The reason I ask is that I thought I remembered +Maile Ohye say that both were not needed and could potentially cause confusion. Appreciate the help!
