    Social Services Coordinator, 2008 - 2010
    • Provided education, assessment, prevention and case management to members of a Managed Care Organization in the following areas: physical health, psychosocial health, environmental safety, and palliative care. • Created outcome-based Member Centered Plans which guided budgetary decisions related to individual capitation rates set by Medicare and/or Medicaid. • Worked within strict deadlines set by the state of Wisconsin which guided the delivery of covered services. • Created training modules and materials for many different types of care providers. • Provided crisis intervention for suicidality, Adult Protective Services, housing issues, H1N1 cases, and hospitalizations. • Developed and implemented a new Behavioral Health Team which served five Wisconsin counties. • Collaborated with hospitals and nursing homes for care and discharge planning. • Located alternative, community-based placement as well as made rehabilitation referrals (inpatient and outpatient). • Maintained detailed electronic medical records, documented case notes per state standards, and kept direct time reporting on all member related activities according to HIPPA guidelines. • Provided two month, six month, and annual reviews for each member along with quarterly visits and visits as needed. • Executed Long-Term Care Functional Screens which determined eligibility and Level of Care for each member. • Assessed for, purchased, and delivered Durable Medical Equipment and Durable Medical Supplies. • Monitored for abuse, neglect, and falls and reported to proper authorities when the situation warranted. • Worked on call to ensure member needs were met such as medical and non-medical transportation, suicide intervention, provider concerns, and other emergencies. • Utilized interdisciplinary collaboration including: Nursing, Purchasing, Providers, Provider Relations, Compliance, other teams, and clinical supervisors.
