DRY EYE PRECAUTIONS

Strain / Irritation / Watering eye is caused by dry eye & allergy problem which is increased by following :- (So take precautions according)
1. More Evaporation of tears or less tear secretion. Wind, dust, plant pollen exposure to be avoided.
2. Dry air exposure (Heater /A.C- both can affect).
use Glasses Full time or / Photochromic Goggles full-time.
3.Infrequent Blinking of Eyes . Reduce TV / Mobile game / Computer / Laptop use.
4.Regular use of goggles /sun-glasses/glasses helps to avoid exposure.
Always use Helmet visor if driving / riding scooter /two wheeler
Avoid car A/C or heater air at face . Keep car windows closed
Keep eyes closed if not driving yourself whenever possible.
5.Avoid medicines for High B.P like Beta Blockers eg.
Atenolol (AT), Metoprolol, Propranolol, Clonidine, etc.
6.Avoid plan /joint pain medicines like: Ibuprofen / Ketoprofen, Brufen, Indomethacin/ Diclofenac etc. to be avoided
7.Anti-Depression medication , Rheumatoid , Arthritis , Thyroid disorders & some Acnae medicines also increase dryness of eyes.
8.In ladies after hormone changes of menopause dry often occur.
9.Avoid eye drops with strong preservatives.
10.Do not wash too frequently , Do cold compresses instead. May alternate with warm compresses if eye lids mebomian / oil gland dysfunction

WINTER EYE CARE TIPS
Many times, we do not realise that a sudden drop in the outside temperature especially during winters can pose a threat to our eyes. Most of us think that eye care is not necessary during the cold and it should only be considered during the summer and monsoon season. However it is to be noted that the sun’s influence remains throughout the year and winter is no different. Exposure to UV rays during winter can in fact pose temporary harm to the eyes and this is why eye care is essential. SNOW BLINDNESS (Medical :Photokeratitis) is a painful condition that can be prevented by using sunglasses or eye protection that transmits 5–10% of visible light and absorbs almost all UV rays. Additionally, these glasses should have large lenses and side shields to avoid incidental light exposure. Sunglasses should always be worn, even when the sky is overcast, as UV rays can pass through clouds The cold wind and dry air can cause irritation to the eyes especially while you are outdoors.
When it comes to eye care during winters, there are no different ways of protecting your eyes. Here are a few ways worth considering:
• Indoor heaters when used tend to eliminate the moisture from the air which can easily dry out eyes and cause irritation. It is recommended to use a humidifier in your room when the humidity factor is considerably low. This would help moisten your dry eyes especially when they are exposed to warm air.
• If in case you live in snowy regions or if you intend to spend your winters at a hill station where there are chances of snow then it is advisable to wear sunglasses that have the ability to block 99 to 100% UV light. Most people tend to forget that the sun is just as bright when it gets reflected on the snow or there can be intense UV light exposure even on a cloudy day when visible light is less but UV Light is comparatively more. Good quality UV 400 protecting sunglasses are essential for quality eye care. The glasses can also prevent and reduce exposure to both the cold and the wind.
• Those who wear contact lens should try to limit outdoor exposure and also use artificial tears more frequently. During winters (just like summers) they would need considerable moisture and in case they start to dry out, the lenses can change shape and even stick to the eye. This can cause pain and cloudiness and drinking alcohol can also increase the problem.
• Seeing is believing and what you get to see these days is no hogwash. As you run your eyes through your surroundings don’t be surprised if everything seems blurry. It’s not your mind playing tricks but the thick layer of deadly smog that is the culprit. Smog, what relation does it have with your eyes, you’d wonder? It’s only respiratory ailments or skin allergies that are caused by smog is a common belief. Well, the lethal combination of fog and smoke – smog – can affect your eyes too.
• Drink a lot of water. Staying hydrated is essential for aiding in adequate tear formation. It becomes all the more essential when external factors like smog increase your proneness to dry eyes. Refrain from stepping out during peak smog hours like early mornings. It’s best to stay indoors during this time
• While using public or private transport, try not to expose yourself to the pollution and smog. Rolled up windows can be a saviour
• Consume food rich in Vitamin A. These include green leafy vegetables, especially spinach which is high in vitamin. Bright-coloured vegetables like yellow pepper and carrots too are beneficial for your eyes
• Ask your doctor for lubricating drops and / or drops which can help increase tear formation

Latest Diagnostic and Eye Laser Treatment equipment and facilities with most experienced doctors for the best Treatment of Diabetic Retinopathy. Some guidelines for patients are as follows:-

A strict metabolic control of blood sugar level along with strict control of lipid profile, renal functions (urine microalbumin, serum creatinine) and B.P. very important. All diabetic patients should have regular eye tests (every 6 months / yearly) by a retina specialist even if they do not feel any eye problem of their own. Those who have developed diabetic retinopathy may need more frequent check-ups. The long term effective treatment is LASER photocoagulation. This is usually undertaken after the test of Fundus Fluorescein Angiography (F.F.A.) and O.C.T. (Optical Coherence Tomography). Both our hospitals have the most advanced such equipment in the region. Laser machines emit a special type of laser light beam which helps to coagulate or seal any bleeding or leaking blood vessels in the retina. Laser treatment (photocoagulation) is beneficial in minimizing further visual loss. New automated pattern multispot laser machines in our hospital (PASCAL LASER) have made retinal laser treatment painless, faster and more precise. As and when diabetic retinopathy progresses. further sessions of laser photocoagulation may be required. If the bleeding inside the eye is too extensive for direct laser treatment and the same does not clear off on its own within a few months, the blood and fluid inside the eye have to be removed by the major operation of vitrectomy and endolaser. Anti-VEGF injections like Lucentis, Accentrix, Macugen, ( Avastin now banned) or IVTA , Ozurdex implants are at times used to control leaking blood vessels (neovascularisation) and swelling inside the eye (macular edema). These are given as injections directly inside the eye with all aseptic precautions in OT. Laser or Anti-VEGF treatment does not guarantee that diabetic retinopathy can be cured, it can only try to reduce the complications and save at least some useful vision in patients who if left untreated are at a very high risk of going blind. Alcohol and Tobacco use are also great compounding factors in this disease. As for many other diseases regular preventive check-ups & timely treatment play a major role in treatment of Diabetic Retinopathy which is fast emerging as the leading cause of blindness worldwide.

Sleeping on a wedge-shaped pillow that elevates the head 20 degrees reduces nighttime intraocular pressure (IOP) and thereby might help control or reduce the risk of glaucoma.

Fifteen people with self-reported glaucoma and 15 without glaucoma were evaluated in a sleep laboratory on two occasions: in one session, they slept lying flat on their backs with no pillow; in the second session, they slept on a wedge-shaped pillow that elevated their head 20 degrees from the surface of the bed.

IOP did not differ significantly between the two positions during the initial (awake) measurement for either group. But during the sleep period, the mean IOP reading when subjects slept on the wedge pillow with their head raised was 1.56 mm Hg lower in the glaucoma group and 1.47 mm Hg lower in the normal group, compared with sleeping flat on the bed.

This corresponds to a 9.3 percent reduction of IOP in the glaucoma group and an 8.7 percent reduction of IOP in the non-glaucoma group, compared with measurements taken when subjects slept on their backs without the pillow.

A total of 25 of the 30 subjects (83.3 percent) had lower eye pressure when sleeping with their head elevated, and 11 subjects (36.7 percent) had reductions in mean IOP that exceeded 10 percent when sleeping on the wedge pillow.

The study authors concluded that sleeping with the head elevated 20 degrees reduces nighttime IOP measurements in glaucoma and non-glaucoma subjects alike, compared with sleeping in the supine position with a flat back.

LASIK and other types of laser eye surgery, such as PRK and ASA or LASEK, have excellent safety profiles and very high success rates and are designed to treat all types of glasses numbers of minus or plus, with or without cylinder, myopia (nearsightedness), hyperopia(farsightedness) and astigmatism.

As per the statistics given on various US websites, LASIK outcomes have been overwhelmingly positive. Most US military patients see 20/20 or better after the procedure without corrective eyewear, and the rate of complications has been very low. According to one study, Only one in 112,500 patients required medical disability retirement due to complications from laser vision correction during this eight-year period.

In another LASIK results study, 100 percent of pilots and other aircraft personnel from the U.S. Navy and U.S. Marine Corps who underwent custom (wavefront-guided) LASIK attained 20/20 uncorrected visual acuity within two weeks after treatment.

To quote from the FDA News Release of September 13, 2016

“The U.S. Food and Drug Administration today approved the VisuMax Femtosecond Laser for the small incision lenticule extraction (SMILE) procedure to reduce or eliminate nearsightedness in certain patients 22 years of age or older.” (Limited number and no cylinder or astigmatism)

As per FDA the approval is with a with a strict note of caution that not all patients are candidates for SMILE, and individuals should carefully review the patient labeling and discuss their expectations with their eye care professional.

As per the FDA approval note a clinical study of the safety and effectiveness of SMILE to correct nearsightedness (Myopia only) found the procedure resulted in stable vision correction at six months. Long term results are not known. Of the 328 participants evaluated at six months, all but one had uncorrected (without glasses or contacts) visual acuity of 20/40 or better, and 88 percent had uncorrected visual acuity of 20/20 or better. That is a significant number (12%) participants did not get 20/20 even in the better eye (as vision of both eyes together is reported).

As per US-FDA common complications during SMILE surgery included difficulty removing the corneal tissue and loss of suction that keeps the laser aligned with your eye. Common complications after surgery included debris at the site of tissue removal, dry eye, moderate to severe glare and moderate to severe halos.

Comparatively in LASIK with excellent Eye Pupil Centeration and auto-tracking, Cylinder Axis Torsion Error Correction, modifiable Optic Zones and Aspheric Correction Profile (none of these even possible in SMILE), glare and halos are not there at all or at most mild to moderate. LASIK and it’s various modifications have been in use for at least three decades with excellent visual outcomes and as compared to even the best SMILE treatment. Worldwide, including the U.S., LASIK or preferably ASA / Epi-LASIK are still the gold standard treatment for laser vision correction.

Diabetes Mellitus (Sugar disease) is a common community health problem.
From an estimated 30 million patients in India alone. the figure is expected to increase up to 60 million cases by the year 2010. It causes swelling and bleeding in the Retina (Parda) of the eye along with growth of abnormal new blood vessels which cause more complications. The damage is frequently permanent and hence needs prompt treatment. About one-fifth of all adults found to diabetic for the first time have already developed this complication. By twenty years of the disease nearly 100% of patients with Type I diabetes (childhood onset type) and 60% of Type II diabetes (adult onset type) have developed this complication.

HOW TO USE THE FUSION EXERCISE CARD :-
1.Hold the card / image in your left hand at arm’s length at the same level as your eye.
2.Take a pen or pencil in your right hand (or vice versa if left handed) and hold it in front of the card in between the two incomplete cats.
3.Look at the tip of the pen. Keep concentrating on it and move the pen slowly towards your nose keeping the card still till you see a third complete cat in between. It may need some effort & good concentration at the tip of the pen.
4. Once the two images overlap, at that point stop moving the pen (point of focus) any closer and maintain your focus on the pen tip for few seconds so as to keep the central cat complete.
5.After about 10 seconds take the pen back suddenly. Repeat these steps a few times.
6. After doing the exercise relax your eyes by looking into the distance or by closing them for a minute or so.
Ideally only one central complete cat image should be seen. Three images with the two partial and a central complete image is also acceptable. The image may appear slightly out of focus as the concentration is on the pen tip. This exercise is to be done with your glasses on.
