I generally do not bother with accents for NPCs. I do occasionally with characters that I'm playing over the course of a campaign. Mostly because if I'm switching around during a game, it is likely that I'll start randomly changing accent in the middle of a speech and distract myself when I'm trying to convey something to the players.

I do have body language for each NPC, which is just something I developed naturally. It's gratifying the times players have realized who just walked in or who they walked in on based on how I'm holding my head or arms.

It is interesting to think about how some of the stereotypical accesnts for creatures that never were came about.
