Phased combat idea: Roll one d6 for each side (party/monsters) for initiative as usual. If the party wins, they go as usual. If the monsters win by one, the party members with a +1 Dex mod can go simultaneously in first round, if they win by two, +1 and +2 Dex modifiers, and so on (if you use +3 or have some magic item that affects the modifier).

Possible: On a tie, negative Dex mod means you miss the first simultaneous round.

Probable: For the rare monster that actually has modifiers, apply them similarly.

I'm pretty sure I'm not the first with the idea (heck, it may be in a book somewhere, and I'm "inventing it" by dredging up an old memory), but it just lept into my head while writing a business email.
