The only caveat is that you must keep your phone, plan, and carrier for 181 days or you'll have to pay Amazon back for the lost commision. I ended up buying a new phone for $0.01 after the commission discount kicked in off a $599 off-contract phone!
Your plans and monthly payments stay the same. I've used Amazon in the past before to get my upgrade and everything was perfectly smooth to swap my new phone from Amazon at home and nothing about my plan changed.
