This is tooth or cavity decay. It’s a bacterial infection that causes destruction of the hard tissues of the teeth (enamel, cementum and dentin) it also causes demineralization of the teeth. Dental caries are as a result of an acid produced by bacterial fermentation of the food deposits accumulated on the surface of the teeth. The study of tooth and cavity decay (dental caries) is known as cariology. There are various treatments for dental caries depending on the extent of tooth destruction. Signs and symptoms of dental caries.
Signs in the earliest stages cannot be easily seen and majorly people with caries may not be aware of them. The first and earliest of tooth and cavity decay is the chalky white spot that appears at the surface of the tooth. This white spot is referred to as micro cavity or incipient carious lesion. If the white spot goes on to demineralize it turns brown which eventually turns into a cavitation. However, before cavitation the tooth can be regenerated and if a cavity is formed the tooth structure that has been lost cannot be recovered. If dental caries existed on the surface of a tooth, the lesion that was formed appears shiny and dark brown showing demineralization process had stopped, whereas an active demineralization is lighter and dull in color and appearance respectively.
Those areas that are affected are always soft and change in color. Once the enamel is destroyed together with the dentin the cavity is now more noticeable. When the decay goes through the enamel it exposes the dentinal tubules which are connected  to passages of the nerve of the tooth transcending to pain that can be worsening with exposure to heat, cold, sweet foods and drinks. A weakened tooth by serious decay sometimes can suddenly fracture even during normal chewing.
When the tooth decaying goes on to  an extent that the bacteria overwhelms the pulp tissues which is located at the centre of the tooth a toothache occurs with the pain being more constant. This can cause the death of pulp tissue thus infections are very common. The tooth then loses its sensitivity to cold or hot but it remains tender and vulnerable to pressure. Other symptoms for dental caries are bad breath with foul tastes. In severe cases the infection may spread to the nearby surrounding soft tissues leading to complications such as Ludwig angina and cavernous sinus thrombosis that are life-threatening.
